Objective. To study the pelvimetry parameters of the bone pelvis in patients with and without apical prolapse (AP). Materials and methods. A retrospective analysis of the medical records of 280 patients was performed: 130 with AP (study group), 100 without AP (with anterior and/or posterior vaginal wall prolapse; comparison group), 50 without vaginal wall prolapse and AP with or without signs of pelvic floor dysfunction (control group). Results. A significant increase in the anteroposterior and transverse diameters of the wide part and narrow part of the bone pelvis, and a decrease in the angle of the pubic arch were found in AP (p < 0.017). Also, more funnel-shaped configuration of the pelvis (expanded in the pelvic inlet) was found in AP compared to patients without AP. Conclusion. The results of this study indicate that bone pelvis geometry plays a significant role in the pathogenesis of genital prolapse. Widening of the pelvic inlet, narrow and wide parts in combination with a decrease in the angle of the pubic arch and an increase in the steepness of the “funnel” are factors predisposing to the development of AP. © 2025 Elsevier B.V., All rights reserved.
