Rapidly progressive periodontitis. (A successful experience of eighteen years of observation)

Rapidly progressive periodontitis is a variant of the aggressive course of periodontitis, characterized by manifestation under the age of 35 and rapid loss of bone tissue. The prevalence of aggressive forms of periodontitis varies for different populations and reaches the highest values for the population of African content — 4.2%. One of the key microbial factors associated with the development of the disease is Aggregatibacter actinomycetemcomitans, which produces a large amount of leukotoxin that disrupts leukocyte migration and inhibits fibroblast activity. The treatment of such patients is associated with a large number of difficulties, and as a rule has an unfavorable prognosis, however, with timely diagnosis and well-chosen treatment tactics, it is possible to achieve stabilization of the process. A clinical case of successful management of a patient diagnosed with rapidly progressive periodontitis of severe severity; chronic generalized periodontitis of stage III, grade B, for 18 years (from 2006 to 2024) is described. The patient sought help at the age of 20, was given an accurate diagnosis and underwent comprehensive treatment with correction of individual hygiene. For 14 years, the patient did not seek specialized periodontal care and only in 2023 he again came to see a periodontist. At this stage, a comprehensive rehabilitation program has been planned and carried out, including conservative, surgical and supportive therapy. Upon completion of the active phase of treatment, stabilization of the process was achieved. The success of the treatment of rapidly progressive periodontitis largely depends on the early diagnosis of the disease and the establishment of an accurate diagnosis. Only compliance with the recommended regimens of individual oral hygiene and regular follow-up will allow you to maintain the achieved therapeutic results in the long term. © 2025 Elsevier B.V., All rights reserved.
